Expert talk: ESC Guidelines for the Management of CVD and CKD

ESC Congress 2026 | As one of the three newly released ESC Guidelines at the congress in Munich, the Guidelines for the Management of CVD and CKD are the central point of discussion in our expert talk: Prof William Herrington (University of Oxford, UK), Prof David C. Wheeler (University College Hospital, London, UK) and Herzmedizin.de editor-in-chief PD Dr. Stefan Perings discuss the document's implications for clinical care.

Take-aways

  • Early screening is key: Cardiologists should assess kidney function in patients with cardiovascular disease using both eGFR and urine albuminuria, as earlier detection enables earlier treatment and better protection of kidney function.
  • eGFR and albuminuria are cardiovascular risk markers: Lower eGFR and higher albuminuria are associated with increased cardiovascular risk, as well as kidney damage and risk of kidney failure.
  • Start risk-modifying therapies early: The guideline recommends initiating evidence-based therapies early and using eGFR, albuminuria and diabetes status to guide subsequent treatment choices, including ACE inhibitors/ARBs, SGLT2 inhibitors, finerenone and semaglutide.
  • Closer collaboration across specialties is essential: Cardiologists and nephrologists should communicate closely, particularly when kidney function is rapidly declining, the underlying kidney disease is uncertain, or patients with severe CKD require cardiovascular interventions.
